All nominations must be accompanied by a minimum of two letters of recommendation from or organization to be represented, a completed nomination form and any other information regarding the nominee's qualifications.

Nomination forms are available from BLM offices and through its Web site at They should be sent to Bureau of Land Management, Utah State Office at 440 West 200 South, Suite 5000, Salt Lake City, UT 84101. Nomination forms and recommendation letters are due by April 2.
